Abstrakt
First trimester screening constitutes a pivotal component of prenatal care. In the Czech Republic, its implementation has progressively expanded since 2004. The present guideline clearly and systematically delineates the role of first trimester screening within the national prenatal care framework, outlining its principal objectives and benefits — most notably, the accurate determination of gestational age, assessment of fetal number and chorionicity, estimation of the risk of chromosomal abnormalities, and evaluation of the risk for pregnancy-related complications, particularly preeclampsia. Moreover, the guideline specifies the fundamental prerequisites for the appropriate execution of first trimester screening, particularly in terms of ensuring adequate diagnostic performance — sensitivity, specificity, false positive rate (FPR), and the number needed to treat (NNT) to prevent one case of preeclampsia through acetylsalicylic acid (ASA) prophylaxis. 
The publication offers a comprehensive overview of the methodologies employed in conducting each component of the screening process, with references to corresponding source materials. The recommendations are presented in a concise, structured format and are supplemented by citations of relevant primary literature and clinical practice guidelines issued by international professional societies. 
To support the development of these recommendations, key documents issued by the International Society of Ultrasound in Obstetrics and Gynecology (ISUOG) and the International Federation of Gynecology and Obstetrics (FIGO) were translated into Czech. The translation of both documents was verified and approved by ISUOG and FIGO, and they are now accessible to the broader professional community. It is anticipated that the availability of these resources in the Czech language will enhance awareness of the purpose and significance of first trimester screening and underscore the importance of methodological adherence to ensure the anticipated clinical effectiveness.
